The following materials are intended for primary care providers. The slide kit may be used for CME presentations to this audience and is supported by a one page summary as a handout. The algorithm provides an easy visual framework for primary care doctors in their workplace. The eGFR tables will be of use in those practices where laboratory reporting of eGFR is not being done. These tools are able to be downloaded and saved as read-only versions, as they have been created and endorsed by the CSN. Any alteration of these materials without CSN consent is not permitted.
